Each time I fly with my trained service animal, I have to remind myself what the procedure is with each airline.
For reference, Alaska uses a thirdparty provider who clears your animal and gives you a special number to use on subsequent reservations:

Steps:
- Go to reservations.alaskaair.com/reservation
- Enter confirmation number or find existing trip
- Scroll to the desired passenger
- Click "Request assistance" button (or "Edit" if already done)
- Add/delete services for
screen - Check box next to Service Animal
- Click link: "To submit your DOT Service Animal Air Transportation Form online click here"
- Link goes to https://traveller-app.opendoorsnfp.org/home
- First time: Click here to Submit Your Department of Transportation Service Animal Form
- Every time: Notify Airline of Upcoming Travel with Your Service Animal
I'd give this airline a B- for convenience of this process. It's quite a pain initially but at least you don't have to fill out the whole form every time.
