15 hour flight with meds that need refrigeration… how do you handle it?

I’ve seen a few posts about traveling with medication, but nothing for a trip this long.

My long haul is 15 hours, but with two connections it’ll be close to 24 hours total travel time. The medication needs to stay refrigerated.

Has anyone done something similar? What did you use to keep it cold that long?

Also, will flight attendants store meds in their fridge if needed, or is that unrealistic?

Would really appreciate hearing real world experiences before I fly.
